The Director of the Coordination of Care Program is responsible for the development and implementation of programs to improve awareness of hospital services to the community to improve patient care through education and outreach to physicians, care facilities and community services. Hires and manages the work of the Coordinator, Hospital Care Coordination direct reports and assists with problem resolution of escalated issues. Facilitates transfer of patients from care facilities to transfer service.  This position will focus on building relationships for the purpose of increasing the visibility of hospital services. 



Responsibilities

• Develops and implements programs to improve awareness of the hospital’s service lines to the community.  Performs visits to physician offices, senior centers, nursing homes, medical groups, etc. throughout the assigned geographic market to improve patient care in alignment with the overall service line plan.

• Maintains relationships with doctors, facilities, community services to coordinate transfers through transfer services for patient care

• Manage the daily operations of the assigned staff and area, troubleshooting and resolving issues as they occur. Demonstrates strong leadership and collaborative skills. Performs coaching, training, hiring, counseling and performance assessment processes.

• Plans and executes internal and external events and programs designed to enhance awareness of hospital services to the community.  Works closely with hospital staff/leadership to ensure success. Works with administration to inform and create new practices for hospital services based on the needs of the community.

• Serves as the subject matter expert regarding hospital strengths and weaknesses within the assigned geographic area.  Develops a clear understanding and expertise regarding services/programs that support community needs. 



Qualifications

Required Qualifications: 

  • Bachelor’s Degree or Equivalent combination of education and experience 
  • 7 years previous hospital program development experience in areas such as IOP, Bariatric, Cardiac, etc. 
  • Excellent written and verbal communication skills in English
  • Ability to communicate to physicians and co-workers in a concise and effective manner
  • Ability to maintain a work pace appropriate to the workload
  • Ability to work independently with minimal supervision and direction
  • Must demonstrate customer service skills appropriate to the job
  • Demonstrates excellent organizational skills/abilities
  • Ability to establish and maintain effective working relationships
  • Valid California Driver’s License and reliable/insured vehicle

Preferred Qualifications: 

  • Master’s Degree

Pay Rate: Min - $151,000 | Max - $207,650
