WHO YOU ARE: 

Under the direction of the Director of Care Coordinaton, the Deputy Director for Justice Involved Program will provide direct oversight of the implementation and operations of the State and City programs, including the daily activities of the Senior Program Manager, Community Health within each of the primary care clinics, carceral sites, and community-based organizations. The ideal candidate will have experience with advanced case management techniques, including harm reduction strategies, crisis intervention techniques and motivational interviewing, Bilingual candidates are encouraged (English/Spanish or English/French/Haitian Creole).

 

POSITION SUMMARY:

This position is grant funded by HRSA Quality Improvement Fund-Transitions In Care for Justice-Involved Individuals, a program that aims to holistically improve the physical, behavioral, and emotional health and well-being of persons with justice involvement who are returning from New York State prisons (specify locations in WH and LIC) utilizing trained Community Health Workers with lived experience with the justice system. The program aims to increase participants’ access to, engagement in, and quality of community-based healthcare and social services, with engagement beginning up to 90 days prior to release. Community Healthcare Network will leverage its experience over the past five years supporting re-entry for persons returning from City jails.


This position is funded through the HRSA Quality Improvement Fund-Transitions In Care for Justice-Involved Individuals grant. Employment is contingent upon continued funding from this grant. The position is currently funded through 2026, with the possibility of extension based on grant renewal or additional funding.

 

 

Essential Functions:

  • Collaborates with care management leadership and Health Justice Network (HJN) staff in the program development, implementation and iteration of policies, procedures, and programming
  • Collaborates with CHN departments and external partners to increase awareness of program and needs of the justice-involved populations
  • Build relationships with carceral sites and community-based organizations through in person visits and in service trainings
  • Establishes workflows with carceral sites and CHN locations for patient referrals, pre-release planning, SDOH screening, initial assessments, care plan development, and transitions into CHN’s services, post-release
  • Oversees senior program manager’s supervision of program Criminal Justice Community Health Workers
  • Serves as mentor for workforce development projects, working with newly hired staff with prior criminal-legal system involvement
  • Leads individual and co-facilitates case specific supervision and interdisciplinary care conferencing.
  • Coordinates with community partners for development of referral network for SDOH and supportive services, including housing, employment/workforce development, legal services, and food access
  • Manages relationships with key staff at all primary care clinics and community-based organizations
  • Coordinates CHN’s justice-involved programming, including the NYC Health Justice Network (HJN)
  • Works in collaboration with HJN staff to support CHWs in uniform implementation of best practice strategies to promote referrals, coordination of care and monitoring of clients with complex needs, as needed
  • Works closely with supervisor to oversee grant contract, budgets, and administrative needs
  • Track and monitor client outcomes and produce/ maintain detailed, accurate and timely reports on productivity and quality outcomes
  • Provides support for the Monthly Performance Reports – Quantitative and Qualitative Reports
  • Ensures compliance in maintaining participants’ records through data management system, including conducting documentation review for CHWs.
  • Monitors education/training needs; facilitate implementation of identified training needs
  • Communicates with project staff as needed to maintain project fidelity
  • Meets with patients, as needed, for support to senior program manager and at times, CHWs
  • Participates in CHN’s Trauma-Informed Care Committee to ensure representation of client needs in TIC planning
  • Performs other related duties as assigned by supervisor


EDUCATION & EXPERIENCE REQUIRED:

  • Bachelor's degree in Social Work, Sociology, Psychology, Public Health, or a related field or a satisfactory equivalent of education and experience combined
  • Master’s Degree preferred.
  • Minimum of five (5) years of experience conducting care management practices and procedures, including supervisory level experience
  • Experience providing counseling, outreach, workshop facilitation, and/or group/community presentations
  • Experience in case management with highly marginalized populations, particularly those with health, mental health, and substance abuse issues
  • Preference will be given to candidates with lived criminal justice experience
